Information for PASSPORT Service Providers
PASSPORT is a Medicaid waiver program that provides home and community-based services and supports to older Ohioans.
Consumer choice is a driving force of PASSPORT and we actively seek new providers to provide services and supports. We foster a high-quality, cost-effective service network that meets the needs of our consumers, with multiple providers of the same service in a particular geographic area. Providers may apply for certification at any time.
Interested providers can contact their area PASSPORT Administrative Agency to begin the provider application process. All providers must meet the Provider Conditions of Participation and Service Specifications for the service or services they wish to provide, which can be viewed at the Register of Ohio.
