Evidence-based Healthy Aging Programs
Are you tired of feeling sick and tired? Take control of your life and manage
your health conditions with a program in your area. "Evidence-based" programs are proven
community initiatives that provide health benefits and promote disease prevention. For more information on the programs below, Call 1-800-422-1976 or visit www.connectmeohio.org (keyword: healthy aging).
- Healthy U - "Healthy U" is for people with ongoing health conditions, such as diabetes, asthma, emphysema, heart condition, high blood pressure, arthritis and others. By participating in a six week session, you’ll learn how to deal with symptoms, handle frustration and design your own self-management program. Healthy U is available in southeast, central and northwest Ohio and the Akron/Canton area.
- A Matter of Balance: Managing Concerns about Falls - Do you restrict your activity because you are afraid of falling? "A Matter of Balance" can help you see falls as preventable and let you take control of your fear. Through eight two-hour small-group sessions, you’ll learn to set realistic goals for activity, master strength and balance exercises and discover how to change your environment to reduce risk. A Matter of Balance is available in north-central Ohio and the Cleveland area.
- Active for Life® - For some older adults, getting the recommended daily amount of physical activity may be enough to reduce the risk of serious health problems, such as heart disease and stroke. Active for Life® is a 20-week program that will give you the motivation and skills to become more active in your everyday life. Active for Life® is available in southwest Ohio and the Cleveland area.
These evidence-based healthy aging programs are provided through a collaboration of the Ohio Department of Aging, Healthy Ohio, the U.S. Administration on Aging, Ohio's area agencies on aging and local public health and aging service providers.
